How Do I Create An onpatient Account?

To create an onpatient account you will need to contact your healthcare provider and request an onpatient invitation. The invitation will be sent to the email address associated with your patient chart. You can independently create an onpatient account by accessing the onpatient web portal at onpatient.com. If you choose this route you will need to ensure that the email address you are utilizing to create your account matches the one your healthcare provider has documented in your chart. You will still, however, need to request an onpatient invitation from your provider in order to connect their information to your account.

Once you have accepted your provider's onpatient invitation you will need to verify your identity through your Date of Birth, Phone Number, or Social Security Number. Once your identity has been confirmed you will be able to view your health history, schedule appointments, make payments for outstanding balances, and facilitate open yet secure communication between you and your doctor via online and mobile methods.
